1. Please fill out the following Civil Rights Complaint Form, which can also be found at the Cashmere School District Administration Office, located at 210 S Division St, Cashmere, WA  98815.
2. All written complaints will be accepted by the school district and forwarded to the Chief Nutrition Officer of Child Nutrition Services, Washington Office of Superintendent of Public Instruction.
